Re-enrolling students may register at the parent's convenience any time between June 1st and August 1st.

In addition to the 2 registration forms, you may also need to turn in an updated immunization record

or a sport physical/consent form and a copy of your insurance card if your student is planning to play sports.

​These forms and payment must be turned in by August 1st.  Once you have turned in your paperwork

and fees you will receive an email stating that your registration is complete.  

If you have not received an email within a week, please contact the Homeschool Coordinator to inquire.  

All forms necessary for the year are listed and explained below.

ALL required forms are accessible using the link to the cloud drive. Please do not hesitate to contact the Homeschool Coordinator with any questions.

1. Curriculum Overview: This form must be printed off, completed, and turned in to the Homeschool Coordinator's mailbox.  If you have any questions regarding curriculum please email the Homeschool coordinator.

2. Financial Enrollment Contract: This form must be printed and completed to be turned in to the Homeschool Coordinator's mailbox.  You will need to use one form for each student you are enrolling.  The Fine Arts fee is optional and is further explained in the Homeschool Overview. Please pay your fees by check or money order.  Full payment for the year is required to complete registration. 

3. Attendance/Grades:This form is for you to use during the year to keep track of the days you have conducted at least 4 hours of school. 

The State requires a minimum of 180 days of school each year. These days can begin July 1st and must be completed by June 30th. This form must be turned in to the Homeschool Coordinator by June 30th to complete your file for the year. Not all curriculum comes with a program to determine a quarterly and semester grade, therefore it is up to you as the teacher to determine the student’s grade for each quarter. This form is only turned in at the completion of the required 180 days, no later than June 30th. 

A $20 late fee will be charged for each week that you are late turning in this form. 

If you are homeschooling several students, you must turn in a separate form for each of them.

4. Parent/Teacher In-Service Form: This form is for parents to use during the year for Grades 6,7 and 8 to show different ways that they have improved their knowledge through the year to better teach their student(s). It may be an article, convention, website, book, Podcast, etc that helped to better equip you as a teacher. This form will be due on June 30th, along with your Attendance/Grades form to complete your file for the year. 

5. Sports: A Physical Form is required for any students planning to play sports.

6. Immunizations: If these records have been updated, please bring in a copy of the updated records.
